help with ports n dedicated server

For the past few days I've been playing with some friends on my own dedicated server.

It worked fine for 4 days and then just suddenly stopped working. Nobody except people on my lan could join. The server shows up as having stable latency and looks exactly like it should to people on my lan. To outsiders its at 2000 with no players.
I've spent around 8 hours trying to figure out why and nothing appears to work.
I don't have a dynamic IP.

Here's my details:

ports forwarded on 27015 and 2016 / TCP and UDP on my internal ip
ports allowed through firewall both inbound and outbound
Server updated with steamcmd
I'm using a netgear4500 router

I'm at a loss at this point and this here is my last go at it.

I don't want to host Hamachi so please no suggestions of that.

any other suggestions will be appreciated.

Alkaline:
If you are using a modem which goes to your router, forward the ports on it and check its settings.

EDIT: To access your modem, look for its gateway ip, unplug your router from the network and then connect your PC directly to your modem, finally access through the browser. Or if you have Win7, I think you can access your modem from the Network control panel.

Sorry we aren't using a modem.

I haven't reset my router yet, Ill see if that changes anything.

Tried hosting on a different port?

Perhaps your ISP caught on and blocked the port. I know that some ISPs do this.
