Nice, except I think you should change the "video" part to either "proof" or "video/pictures because pictures of in-game will also count as proof.

If a video isn't available, is it appropriate to post a Log of the Events from the console? I can't see why this would be a problem as it shows the Perpetrator, Witnesses, Events and Actions. (Ill edit this part out after its answered) Would someone please post somewhere how to get steam ID's of a player? a surprising number of people do not realise how to do this. Thanks
