This error where shit starts ungrouping for no reason has occurred to me.
I noticed it only happens when I am in Counter-Strike: Source.

Save your map.
Close Hammer Editor.
Close Source SDK.
Close Steam.
Restart your PC.
Open Steam.
Open Source SDK.
Open Hammer Editor.
Load your map.

If shit is still ungrouped,
open the map under Episode Two and it should be fixed.

If not,
Hammer Editor is trolling.

Yea I was actually going to try it in ep2 as well, but I kind of gave up. I think I'll try it later.
Also, the ungrouping is never permanent. When ever I restart hammer it fixes, but when I do that the thing I had been working on gets removed due to errors :l.

Solution for the displacement problem:

Delete all displacements.
Make new displacements.
