You're missing some key points from this application, namely your STEAMID.

Fixed, but coolzeldad said it looked OK when I sent it to him on Steam. (It was late when I posted this, I had my steam ID ready from that site, must of copied from the wrong box or something)

I've known boncey for a good while. He dosen't minge around on the servers and is pretty friendly, +1 from me.

Although, the application could be improved by following http://forum.randomgs.com/index.php/topic,13489.0.html. But that's completely up to you.
