At least one of you has added bans without adding a trigger.

Examples:

http://forum.randomgs.com/index.php?action=admin;area=ban;sa=edit;bg=10 (http://forum.randomgs.com/index.php?action=admin;area=ban;sa=edit;bg=10)
http://forum.randomgs.com/index.php?action=admin;area=ban;sa=edit;bg=18 (http://forum.randomgs.com/index.php?action=admin;area=ban;sa=edit;bg=18)

YOU NEED TO ADD AT LEAST ONE TRIGGER

I highly suggest only username.
If not that then email.
If not that then IP/hostname.
