I have one, it looks like a human head.
I never play it because it leaves a nasty taste in my mouth and is out of tune.

My friend got a zelda themed ocarina with a booklet on all the music for ocarina of time. She plays it from time to time, it's not really that hard if you have a booklet and have the simple tunes worked out.
